MEMBERSHIP APPLICATION SUBMISSION

In order to apply for PACCC membership, please complete the application form and submit all other required documents.  Options you have in submitting these materials are as follows:

 

FAX:          Download, complete, sign and fax all application information to the PACCC National Office at (866) 263-5347

MAIL:      Mail in with all requested documents to the PACCC National Office at:  PACCC National Office, Box 93, Norquay, SK  S0A 2V0

 

Payment of fees can be made by mailing your cheques (made out to PACCC) to the National Office listed above.  You can also request to pay by credit card or paypal (please ask the Administrator for details).
